Troubleshooting Form Submission Issues on Client's Website

Hello, I'm having issues with a form submission for my clients website. Specifically the submit form button is not showing the success pop up when clicked on. What could be possibly causing this? Thank you

It seems to be set up correctly. I don't think forms are submitting when testing in the Webflow Designer. Have you already tested the form on the published version (example.webflow.io) link?
