Can't make this with hover or indicate that I click on the right one.

2. I want to make more steps then four, for UX. but not sure how it will indicate on the form table

3. Also on the number buttons it's not indicate or hover or selected

4. I want to design the success page too and add back to homepage button

5. I want to add input and make it work on so I can see it on my form table
